Established in 2014 as a unique interiors and lifestyle shop in Milwaukee, WI, Form has grown into a full service interior design firm. With a passion for creating inspiring spaces and innovative design, Form believes in the story a space can tell.

As a self-represented small business, Form takes clients on a limited basis. We work to improve both residential and commercial spaces whether you are looking for a thoughtful refresh or are planning a design from the ground up.

Our services include :

Full Service Design: Intended for clients with large-scale projects who are looking to Form to design and execute their job from start to finish. New Build + Remodel Design is best suited for construction projects where Form is managing all aspects of the design process for the majority or totality of a home (i.e. large-scale renovations and new builds) or Full Home Furnishings for the majority or totality of a home. This also includes partial remodels and or the complete furnishing of one or more rooms in a home.

Styling: Intended for clients who have most of their furnishings in place, but need help from an expert in taking their project to the finish line with selections of decorative details and accent furniture to tie their room(s) together and bring their vision to life. This service is ideal for clients who don’t need a full room(s) designed, but need help evaluating which pieces will work best to round out their space(s) before purchasing.

Commercial Design : Intended for clients that would like to see the vision and soul of their business come to life through interiors. Form is passionate about both refreshing commercial spaces and building them with clients from the ground up.

Our holistic approach to the design process ensures that we will provide design and project management services through all phases of any project, from initial design concepts, through construction design documents, ordering, procurement, fulfillment of all products, and final install.
